I usually buy the t-shirt, print out what I want on fabric transfer paper, then iron it on - that way I can get 100% cotton in the t-shirt and not be itchy in polyester. Print shops do this pretty cheaply, too, and their work is pretty good quality, stands up to washing and drying with no problems.
